We’re proud to be a designated Blue Cross Blue Shield of Michigan Patient-Centered Medical Home practice since 2014. To earn this special designation, we’ve instituted a range of capabilities that help us better manage your care. 



We’re committed to placing the focus on you, our patient. Instead of just treating you for a problem here and there, our patient-centered medical home works to coordinate your care and keep you as healthy as possible. 



Our team of health care professionals is dedicated to improving your overall health and to helping you reach your health goals. This team may consist of a primary care doctor, nurses, specialty physicians, pharmacists, nutrition counselors, care coordinators and others, depending on your needs. 



We’ll be sure to follow up with you after your office visit to see how you’re doing or remind you to schedule any necessary tests to help us manage your condition.



You’ll also have 24-hour access to someone in our practice. That means that if you have a serious health problem in the middle of night or on a weekend, your patient-centered medical home team is available — which could help you avoid a trip to the emergency room.
